These forms are formatted as Acrobat files, a format that can be viewed in any platform. This means you will need the Acrobat Reader application. If you do not have it currently as a helper application for your browser, you can download it free from the Adobe web site. 

If you prefer we fax the forms to you, send us a note with your fax number, we will be happy to forward them to you.

Registration Forms
This document includes the Registration forms needed for enrollment. This is a complete package. For other individual forms see below.

Curriculum List
This is a list of Curriculum that you you will be teaching your child.  To insure accuracy in your student's transcripts, a curriculum list needs to be submitted to our offices for each child that is a member of FCA. This form must be submitted within 30 days of enrollment.

Attendance Reports
Each family shall be required to submit a semi-annual report summarizing the work of each student on a subject-by-subject basis. The report contains written documentation of the days present and the days absent with a cumulative total of days present to date. These are due on or before January 15th and July 15th.

Fees Schedules
The fee schedules for Plans A, B, and D are included, along with a description of the benefits each offers our members.

Requests Form
This form is used for requesting transcripts of grades, copies of your child's Attendance and Progress reports, school records, and Driver's License forms.

Driver's License
To receive the Driver's License forms (SF1010) you need to use our Request Form above. (See requirements for Driver's License compliance.)

Religious Exemption
This form is used to exempt your children from the required Stanford Achievement Tests based on your religious beliefs.

Curriculum Catalog
Used for ordering curriculum for your student.

FCA Coordinators
We are looking for families in specific areas to act as coordinators for our school. There are several different needs that we will be able to meet with your help, with no increase in fees. Extra benefits will be available for those who wish to help.
